About this test

This molecular test looks directly for the genetic material (DNA) of the two commonest bacterial causes of sexually transmitted infection โ€” Chlamydia trachomatis and Neisseria gonorrhoeae โ€” in a single sample. Because it uses PCR (a nucleic-acid amplification method), it is far more sensitive than older culture or microscopy methods and can detect even a small number of organisms before symptoms appear. The sample is usually first-catch urine or a swab from the cervix, vagina, urethra, throat, or rectum, depending on the site of possible exposure. Doctors order it when there is unusual genital or urethral discharge, burning on passing urine, pelvic or testicular pain, bleeding between periods or after sex, or simply after unprotected intercourse with a new or untreated partner. It is also recommended in pregnancy, before procedures such as IUD insertion or termination, and as part of an infertility work-up, since both infections are major treatable causes of blocked fallopian tubes, ectopic pregnancy, and pelvic inflammatory disease in women, and of epididymitis in men. A crucial point in Nepal: both infections are frequently silent โ€” most women and many men feel completely well while still able to pass the organism on โ€” so a negative result is genuinely reassuring while a positive one usually means the partner needs treatment too. Results are read alongside the clinical history; if gonorrhoea is found, a separate culture may be advised so antibiotic sensitivity can be checked, as drug-resistant gonorrhoea is an increasing global concern. The test does not screen for syphilis, HIV, or other infections, which may be tested separately.

Preparation

No fasting needed. For a urine sample, collect the first part of your stream and do not pass urine for at least one hour beforehand. Avoid vaginal washes, creams, or antibiotics in the days before the test, and tell us if you have started any antibiotic recently.
