About this test

Gonorrhoea microscopy is a rapid, low-cost examination in which a swab of genital discharge is smeared onto a slide, Gram-stained, and viewed under the microscope to look for Neisseria gonorrhoeae โ€” the bacterium that causes the sexually transmitted infection gonorrhoea. The classic finding is Gram-negative diplococci (kidney-bean-shaped pairs) sitting inside pus cells, and when these are seen in a man with urethral discharge the result is highly reliable, allowing treatment to begin the same day. A doctor usually orders this test when there is a thick yellow or green discharge from the penis or vagina, burning on passing urine, pain or bleeding between periods, or unexplained pelvic pain, and it is part of the standard work-up for any suspected STI alongside tests for chlamydia, syphilis and HIV. It is also requested for a newborn with red, sticky, discharging eyes, where gonococcal conjunctivitis must be excluded urgently. Microscopy is most sensitive on male urethral samples; in women and at throat or rectal sites the organism is harder to see, so a negative smear does not rule out infection and culture or a molecular (NAAT) test is advised to confirm. Because gonorrhoea in Nepal and worldwide is increasingly resistant to common antibiotics, a positive microscopy result is ideally followed by culture and sensitivity testing to guide the right drug. Results are normally interpreted together with your symptoms, your partner's status, and the other STI screening tests, and treating sexual partners at the same time is essential to prevent reinfection and complications such as infertility.

Preparation

No fasting needed. Men should ideally not pass urine for 1โ€“2 hours before a urethral swab, as recent urination washes away the discharge we need to examine. Avoid douching, antiseptic creams or starting antibiotics before the sample is collected, as these reduce the chance of finding the organism. Tell us if you have already taken any antibiotic in the past week.
