About this test
A Gram stain is one of the oldest and most useful tricks in the microbiology laboratory: a thin smear of your sample is fixed to a glass slide, stained with a sequence of dyes, and examined under the microscope. Within an hour the technologist can tell your doctor a great deal before any culture has had time to grow. Bacteria divide into two broad families by how they hold the dye โ Gram-positive organisms appear purple, Gram-negative ones appear pink โ and their shape (round cocci in clusters or chains, or rod-shaped bacilli) narrows the list of likely culprits further. The smear also reports whether pus cells are present, which confirms a genuine infection rather than harmless contamination, and may reveal yeast or, in a sputum sample, hints that point towards tuberculosis (though acid-fast staining is the specific test for TB). Doctors order it on pus from an abscess, an infected wound swab, sputum in pneumonia, urine in a urinary infection, or fluid drawn from a joint, the chest, or the spinal canal in suspected meningitis. It guides the choice of an initial antibiotic on the same day, while the formal culture and sensitivity โ which takes a couple of days more โ confirms the exact organism and which drugs will work. Because antibiotics are so freely available across Kathmandu, collecting the sample before treatment begins makes the result far more reliable. The Gram stain is a rapid first read, always interpreted alongside your symptoms and the follow-up culture.

Preparation
No fasting needed. For sputum, give an early-morning deep-cough sample, not saliva. For a wound or genital swab, avoid applying antiseptic cream beforehand, and ideally collect before starting antibiotics so the bacteria are still visible.
