About this test
This test looks directly for Helicobacter pylori, the spiral bacterium that burrows into the stomach lining, by detecting its antigen (protein) in a sample of your faeces. A positive result means an active, current infection rather than simply past exposure, which is why it is preferred over older blood antibody tests for both diagnosing infection and confirming that treatment has worked. Doctors typically order it when a patient complains of persistent burning upper-abdominal pain, bloating, nausea, early fullness after eating, recurrent heartburn, or has been found to have a gastric or duodenal ulcer on endoscopy. H. pylori is the single most common cause of peptic ulcer disease and a recognised risk factor for stomach cancer, so eradicating it matters well beyond settling day-to-day dyspepsia. The infection is extremely common in Nepal, where crowded living conditions and contaminated drinking water allow it to spread early in childhood, and many adults carry it without ever feeling unwell. The stool antigen method is non-invasive, needs no swallowing of tablets or endoscope, and works equally well in children and adults, which makes it especially practical for families. It is usually interpreted alongside your symptoms, any endoscopy findings, and a CBC if anaemia from a bleeding ulcer is suspected. After a course of antibiotics, the same test is repeated at least four weeks later to confirm the bacterium has been cleared, since symptoms alone are an unreliable guide to cure.

Preparation
No fasting needed. Stop proton-pump inhibitors (omeprazole, pantoprazole, esomeprazole) for 2 weeks and any antibiotics or bismuth for 4 weeks before testing, as these suppress the bacterium and cause false-negative results. Collect a fresh stool sample in the clean container we provide and bring it to the lab promptly.
