About this test

This test hunts for bacteria living in the seminal fluid and, when an organism is found, works out which antibiotics will clear it. The semen is incubated on culture plates; any significant growth is identified by species and then tested against a panel of drugs, so the report tells your doctor both the culprit and the cure rather than leaving treatment to guesswork. It is ordered when a man has symptoms pointing to infection of the prostate, seminal vesicles or epididymis โ€” pain or burning on ejaculation, ache in the perineum or testicles, blood in the semen, pus-like discharge, or recurrent lower urinary symptoms that ordinary urine tests have not explained. It is also a routine part of the male fertility work-up, because silent genital-tract infection can lower sperm count, impair motility, raise the white-cell count in semen, and contribute to a couple's difficulty conceiving. Common findings include E. coli and other coliforms, Enterococcus, Staphylococcus and occasionally sexually transmitted organisms. Because semen naturally carries some skin and urethral commensals, the laboratory weighs the type and quantity of growth before calling a result truly significant, which is why a clean mid-stream collection matters so much. Results take up to seventy-two hours, since the plates must be watched for slower-growing germs before sensitivities can be added. Your doctor reads it alongside a semen analysis, a urine culture, prostate examination and, where relevant, screening for sexually transmitted infections, so that the right antibiotic is matched to a genuine infection and a partner can be treated when needed.

Preparation

Pass urine and wash the hands and genital area with plain water before collecting. Abstain from ejaculation for 2โ€“3 days beforehand, and tell us if you have already started any antibiotic, as this can suppress growth. The sample is collected by self-stimulation into the sterile pot we provide and should reach the laboratory within an hour.
