About this test
When a joint becomes suddenly hot, swollen, intensely painful and difficult to move, the most urgent question your doctor must answer is whether infection has taken hold inside it โ and this test is how that question is settled. A small amount of fluid is aspirated from the joint space and incubated in the laboratory to see whether bacteria grow; if an organism appears, sensitivity testing then determines which antibiotics will kill it and which it resists. Septic arthritis is a medical emergency because pus inside a closed joint can destroy cartilage within days, so the culture guides both the choice of antibiotic and the decision to wash out the joint surgically. The commonest culprits are Staphylococcus aureus and streptococci, but in Kathmandu the picture is broadened by tuberculosis, which causes a slow, smouldering single-joint arthritis (often the hip or knee) that mimics other conditions for weeks โ so mycobacterial culture and sometimes gonococcal investigation may be requested alongside the routine bacterial culture. The aspirate is rarely interpreted alone: it is read together with the fluid cell count and differential, Gram stain, glucose level, crystal examination for gout or pseudogout, blood cultures, and inflammatory markers such as CRP and ESR. Because organisms can be slow-growing or scanty, a final result takes several days, and a negative culture does not always rule out infection if the clinical picture is convincing. Prosthetic joints, recent injections, diabetes, and immune suppression all raise the stakes, making prompt, properly collected sampling essential.

Preparation
No fasting required. The fluid is drawn from the affected joint by your doctor using a sterile needle (arthrocentesis), usually after cleaning the skin and sometimes with local anaesthetic. Where possible, the sample should be collected before any antibiotics are started, as prior treatment can suppress bacterial growth and produce a falsely negative culture. Tell us if you have already begun antibiotics.
