About this test

Aldosterone is a hormone made by the outer layer of the adrenal glands, the two small caps that sit above each kidney. Its job is to manage the body's salt and water balance and, through that, blood pressure: it tells the kidneys to hold on to sodium and let go of potassium. This test measures how much aldosterone is circulating in the blood, and it is almost always read together with plasma renin so the two can be expressed as the aldosterone-to-renin ratio (ARR), the single most useful screen for primary aldosteronism (Conn's syndrome). Doctors order it most often when high blood pressure is hard to control despite three or more medicines, begins at a young age, or comes with unexplained low potassium that shows up as muscle weakness, cramps, excessive thirst, frequent urination, or palpitations. It is also requested to investigate the opposite picture — low blood pressure, salt craving, fatigue, and darkening skin — which can point to adrenal insufficiency. Because posture, the time of day, salt intake, pregnancy and a long list of common drugs all shift the result, careful preparation and an accurate note of your medications matter as much as the blood draw itself. In Kathmandu, where hypertension is increasingly diagnosed in adults under fifty, identifying a treatable hormonal cause early can spare a patient years of poorly controlled pressure and reduce the long-term risk to the heart, kidneys and brain. Results are interpreted alongside renin, potassium, sodium and, where needed, follow-up confirmatory tests or adrenal imaging.

Tell us if you take blood-pressure medicines, water tablets (diuretics), liquorice, or contraceptive pills — some need to be paused for 2–6 weeks first, only on your doctor's advice. The sample is best taken in the morning after you have been upright (sitting or standing) for about two hours; let us know if you have been lying down. No fasting is needed.
