About this test

This test detects antibodies your immune system makes against thyroid peroxidase, an enzyme the thyroid gland uses to manufacture its hormones. When these antibodies are present in raised numbers, it usually means the immune system is mistakenly attacking the thyroid — the hallmark of autoimmune thyroid disease. The single most important reason a doctor orders Anti TPO is to confirm Hashimoto's thyroiditis, far and away the commonest cause of an underactive thyroid (hypothyroidism). It is also positive in many people with Graves' disease, the leading cause of an overactive thyroid. Your doctor will typically request it alongside TSH and free T4 when those results are abnormal or borderline, to work out whether the underlying problem is autoimmune and how likely it is to progress. Symptoms that often prompt testing include persistent tiredness, unexplained weight gain or loss, feeling cold, dry skin, hair thinning, constipation, low mood, an irregular or heavy menstrual cycle, or a visible swelling at the front of the neck (goitre). The test is especially valued in women, who develop autoimmune thyroid disease several times more often than men, and in those planning pregnancy or being investigated for recurrent miscarriage or infertility, since raised antibodies carry a higher risk of thyroid problems during and after pregnancy. In Kathmandu, where dietary iodine intake has shifted over the years and goitre remains familiar to many families, Anti TPO helps distinguish an autoimmune cause from other thyroid disorders. A positive result is interpreted together with your hormone levels and symptoms; once the diagnosis is established, this antibody is not usually repeated to monitor treatment.
