About this test

Angiotensin Converting Enzyme is an enzyme found mainly on the inner lining of blood vessels, with the lungs holding the largest share. In healthy people it plays a quiet role in regulating blood pressure, but in laboratory medicine its real value is as a marker of granuloma activity — the small clusters of immune cells that form in certain inflammatory diseases. The cells lining these granulomas secrete extra ACE into the bloodstream, so a raised serum level can reflect how much active granulomatous tissue is present. By far the most common reason your doctor orders this test is suspected sarcoidosis, a condition that causes swollen lymph nodes in the chest, a dry cough, breathlessness, skin nodules, eye inflammation or unexplained fatigue. Because tuberculosis is widespread in Nepal and can mimic sarcoidosis closely on a chest X-ray, ACE is one of the supporting tests doctors use to help tell the two apart, alongside imaging, a tuberculin or IGRA test and sometimes a biopsy. ACE is not a stand-alone diagnosis: leprosy, certain fungal infections, an overactive thyroid, liver disease and diabetes can also nudge the number, while ACE-inhibitor medicines push it down and can mask a true rise. Where it shines is in follow-up — once sarcoidosis is confirmed, falling ACE levels often signal that the disease is settling or that steroid treatment is working, while a rising level may warn of a flare. Your physician will always read the result against your symptoms, scans and overall clinical picture rather than the number alone.

Preparation

A 10-12 hour overnight fast is preferred, as a heavy or fatty meal can cloud the sample and interfere with the reading. Water is fine. If you take an ACE-inhibitor blood pressure medicine such as enalapril, lisinopril, ramipril or perindopril, tell us before the draw — these drugs lower the result and your doctor may ask you to pause them beforehand.
