About this test
The albumin/globulin ratio is a simple calculated value derived from your total protein test, comparing the amount of albumin against the combined globulins in your blood. Albumin is made solely by the liver and keeps fluid inside the blood vessels, while globulins are a mixed family that includes antibodies, transport proteins, and inflammatory proteins. A healthy ratio usually sits a little above 1, because albumin normally outweighs the globulins. The number matters less on its own than as a clue to which side of the balance has shifted. A low ratio is the more common abnormality and can point in several directions: chronic liver disease, where the damaged liver makes less albumin; nephrotic syndrome, where albumin is lost into the urine; chronic infection or autoimmune illness that drives globulins up; or, importantly, blood cancers such as multiple myeloma where one globulin band rises sharply. In Nepal, where chronic infections, tuberculosis, and untreated chronic liver disease remain common, a persistently low ratio is a useful early flag that something is quietly out of balance. A high ratio is less frequent and may reflect low antibody production. Doctors order this test when investigating unexplained tiredness, swelling of the legs or abdomen, recurrent infections, abnormal liver tests, or protein found on a urine dipstick. It is never read alone; an abnormal ratio is followed up alongside liver enzymes, kidney function, and often protein electrophoresis to identify exactly which protein is responsible before any diagnosis is made.
