About this test

This test measures antibodies your immune system has mistakenly produced against your own double-stranded DNA — the genetic material packed inside every cell nucleus. In healthy people these antibodies are absent; their presence is one of the most specific markers of systemic lupus erythematosus (SLE), so much so that it forms part of the formal classification criteria for the disease. A doctor usually orders anti-dsDNA after a positive ANA (antinuclear antibody) screen, when symptoms such as a butterfly-shaped facial rash, joint pain, mouth ulcers, hair loss, unexplained fever, sensitivity to sunlight, or kidney problems raise the suspicion of lupus. It is rarely used as a first-line screen on its own; instead it confirms and refines an ANA result and helps distinguish lupus from other connective-tissue disorders. Beyond diagnosis, the test is valuable for monitoring known lupus patients: a rising titre often signals a flare and correlates particularly with lupus nephritis, the kidney involvement that determines long-term outcome. For this reason it is frequently checked alongside complement levels (C3 and C4), urine protein, and anti-Sm antibodies to build a fuller picture of disease activity. Lupus most commonly affects young women of reproductive age, the same group seen often in clinics across Kathmandu, and a delayed diagnosis can allow silent kidney damage to progress. A negative result does not completely rule out lupus, since not every patient carries this antibody, and findings should always be interpreted by your physician alongside your symptoms and other blood tests.

Preparation

No fasting required. Water and your regular medicines are fine. If you are already on steroids or other immune-suppressing treatment, mention this, as it can lower the antibody level.
