About this test

C-peptide is a small fragment released in equal amounts to insulin whenever the pancreas makes the hormone, so measuring it tells your doctor how much insulin your own beta cells are actually producing. This matters because injected insulin contains no C-peptide, allowing the lab to separate the body's own output from any insulin given by treatment — something a direct insulin measurement cannot do. Doctors most often order it to clarify the type of diabetes when the picture is unclear: a low level points towards type 1 diabetes or long-standing type 2 with exhausted beta cells, while a normal or high level suggests insulin resistance, as seen in early type 2 diabetes, obesity, or polycystic ovary syndrome. It also guides whether a person can be managed on tablets or genuinely needs insulin, and is checked in younger or lean patients in Nepal who do not fit the usual type 2 mould. In someone with unexplained low blood sugar (hypoglycaemia), a raised C-peptide alongside high insulin suggests the body is over-secreting — for example from a rare insulin-producing tumour — whereas a suppressed C-peptide with high insulin hints that insulin has been taken from outside. The result is always interpreted together with the blood glucose taken at the same moment, and often alongside HbA1c and fasting insulin. It is occasionally used to confirm complete remission after pancreas or islet-cell procedures. A single value is a snapshot, so your doctor may repeat it or test it after a meal or stimulus to judge your reserve more fully.
