About this test

CA 125 is a protein shed into the bloodstream by certain cells lining the ovaries, the abdominal cavity and the chest, and it is measured most often when a doctor is evaluating an ovarian or pelvic mass. Its single most valuable role is not screening healthy women but monitoring: once epithelial ovarian cancer has been diagnosed, falling levels confirm that chemotherapy or surgery is working, and a rising trend during follow-up can signal recurrence months before it would otherwise be detected. A gynaecologist may also order it when an ultrasound shows an ovarian cyst that looks suspicious, particularly after the menopause, to help judge whether the lump is more likely benign or malignant. It is important to understand what this test cannot do: CA 125 is frequently raised in many harmless and unrelated conditions, including endometriosis, uterine fibroids, pelvic inflammatory disease, ordinary menstruation, early pregnancy, liver disease and even tuberculous peritonitis, which remains common in Nepal and can closely mimic ovarian cancer on imaging. Because of this overlap, a high number on its own never proves cancer, and a normal number does not fully exclude it, so the result is always interpreted alongside a pelvic ultrasound, the patient's menopausal status, symptoms such as persistent bloating, early fullness, pelvic pressure or unexplained weight loss, and sometimes a second marker like HE4. The widely used cut-off is around 35 U/mL, though your doctor weighs the trend over repeated tests far more than any single figure.
