About this test

Follicle Stimulating Hormone is released by the pituitary gland at the base of the brain, and its job is to signal the ovaries or testes to do their work — ripening egg follicles in women and supporting sperm production in men. Measuring it in a single serum sample tells your doctor a great deal about how that signalling axis is functioning. In women struggling to conceive, FSH is one of the first hormones checked, ideally on day 2 or 3 of the menstrual cycle, because a high reading at that point points to a dwindling ovarian reserve, while it is usually read together with LH, oestradiol, prolactin and thyroid hormones to build a complete picture. A markedly raised FSH alongside missed or irregular periods in a woman in her forties or fifties is the classic laboratory confirmation of the menopause, and in younger women it can flag premature ovarian insufficiency. The pattern of FSH relative to LH also helps distinguish polycystic ovary syndrome, a common cause of irregular cycles and infertility here in Kathmandu. In men, the test is ordered for infertility, low libido, shrunken testes or suspected hormonal problems, where a high level suggests the testes themselves are failing and a low level points back to the pituitary. In children it is used to investigate puberty that arrives unusually early or late. Because FSH naturally fluctuates across the cycle and even hour to hour, your doctor interprets the number against your age, sex, symptoms and the day it was drawn rather than in isolation.
