About this test
Free T3 measures the unbound, biologically active form of triiodothyronine — the most potent thyroid hormone and the one that actually drives your metabolic rate, heart rate, body temperature, and how quickly your body burns energy. Most T3 in the blood travels stuck to carrier proteins and is inactive; the "free" fraction is the small portion the tissues can immediately use, which is why it gives a truer reading than total T3 in pregnancy, in women on oestrogen-containing pills, and in any illness that alters protein levels. A doctor rarely starts with FT3 alone — it is usually added to TSH and Free T4 when the first results are confusing or when an overactive thyroid is strongly suspected. It is especially valuable in diagnosing T3-toxicosis, a form of hyperthyroidism where TSH is suppressed and FT4 still looks normal but FT3 alone is raised, and in fine-tuning treatment for Graves' disease. Symptoms that prompt this test include unexplained weight loss despite a good appetite, a racing or irregular heartbeat, heat intolerance, tremor, anxiety, loose stools, and difficulty sleeping. In hospitalised or seriously unwell patients FT3 often falls as part of "sick euthyroid" changes rather than true thyroid disease, so the result is always interpreted in the clinical context alongside TSH and FT4. Goitre and autoimmune thyroid disease are common in Nepal, partly reflecting historic iodine deficiency, so thyroid testing is requested frequently here; FT3 sharpens the picture when the standard panel does not fully explain the patient's symptoms.
