About this test

Luteinising hormone is released by the pituitary gland at the base of the brain and is one of the master switches controlling the ovaries and testes. In women, a mid-cycle surge of LH is the trigger that releases an egg, so the test is central to investigating irregular or absent periods, difficulty conceiving, and suspected polycystic ovary syndrome (PCOS) — a very common cause of subfertility among young women attending clinics in Kathmandu, where a raised LH-to-FSH ratio is a classic supporting clue. In men, LH stimulates the testes to make testosterone, so it is checked when there is low libido, erectile difficulty, infertility, or unexpectedly low testosterone. The test is almost never read alone: doctors interpret it together with FSH, oestrogen or testosterone, and often prolactin, because the pattern matters more than any single number. A high LH alongside low sex hormones usually points to a problem in the ovaries or testes themselves (primary failure, including menopause), whereas a low LH with low sex hormones suggests the pituitary or hypothalamus is not signalling properly. LH is also measured in children being assessed for early or delayed puberty, and to confirm whether menopause has occurred. Because LH is secreted in pulses and swings sharply across the menstrual cycle, the timing of your sample is important, which is why we ask women for their cycle day. A small blood sample taken from the arm is all that is needed, and the result is usually available the same day.

No fasting required. If you are a menstruating woman, tell us the day of your cycle — for example, day 2 or day 3 of your period — because the result is read differently at each stage of the cycle. If you are tracking ovulation, your doctor may ask for samples on specific days.
