About this test
Progesterone is the hormone the ovary produces after it releases an egg, and measuring it in the blood is one of the simplest ways to confirm that ovulation has actually happened. After an egg is released, the empty follicle becomes the corpus luteum and pours out progesterone for roughly two weeks to prepare the womb lining for a possible pregnancy; if no pregnancy occurs the level falls and a period follows. Because the value swings dramatically across the menstrual cycle, the timing of the sample is everything โ a single mid-luteal reading (commonly around day 21 of a 28-day cycle) that is comfortably raised tells your doctor ovulation occurred that month. Gynaecologists most often request this test during the work-up for difficulty conceiving, for irregular or absent periods, and where polycystic ovary syndrome is suspected, usually interpreted alongside LH, FSH, prolactin and thyroid results rather than on its own. In early pregnancy a low or falling progesterone may be checked when there is bleeding or pain to help judge whether the pregnancy is developing normally or there is concern about miscarriage or an ectopic pregnancy. It is also followed during fertility treatment and progesterone-supported cycles. Less commonly it is measured in men and in children to investigate certain adrenal gland conditions. The result is never read in isolation โ your symptoms, cycle day, ultrasound findings and other hormone levels all shape what the number means, so always review it with the doctor who ordered it.
