About this test

Prostate-specific antigen circulates in the blood in two forms: bound to proteins and floating freely. This test measures the free fraction, and on its own it is rarely the whole story — its real value emerges when expressed as a percentage of Total PSA, the free-to-total ratio. That ratio helps your urologist answer a difficult question that Total PSA alone cannot: when the total is mildly elevated, usually in the grey zone of about 4 to 10 ng/mL, is the cause a harmless enlarged gland or something that warrants a biopsy? The biology is reassuringly logical — benign prostatic hyperplasia, the common age-related swelling that makes older men wake to pass urine at night, tends to release more free PSA, so a higher free percentage points away from cancer. Prostate cancer cells, by contrast, produce more protein-bound PSA, dragging the free percentage down. A low ratio therefore raises concern, while a comfortably high one may spare a man an unnecessary needle biopsy. Doctors order it for men whose Total PSA sits in that ambiguous range, often alongside a digital rectal examination and, increasingly, MRI before deciding on biopsy. Because PSA is exquisitely sensitive to recent stimulation, samples taken too soon after sex, a long cycle ride, a catheter, or a rectal exam can mislead — hence the preparation rules. It is interpreted in the context of age, gland size, and any family history, and is never used to diagnose cancer by itself, only to refine risk and guide the next sensible step.
