About this test

PSA Total is the standard screening test for prostate cancer in men and an essential monitoring test in men already treated for it. Prostate-specific antigen is produced by the prostate gland; a raised level can reflect cancer, but also benign prostatic enlargement (BPH), prostatitis, recent ejaculation, or vigorous cycling. PSA screening is most useful for men aged 50–70 (or from age 45 if there is a family history of prostate cancer), in patients with urinary symptoms such as weak stream, hesitancy, frequent night-time urination, or incomplete emptying, and as routine surveillance after prostate surgery or radiotherapy. Doctors interpret PSA in the context of age, prostate size on ultrasound, the free-to-total PSA ratio, and trends over time — a steadily rising PSA is more significant than a single high value.

Preparation

No fasting required. Avoid ejaculation, prolonged cycling, and digital rectal examination for at least 48 hours before the blood draw — all three can transiently raise PSA. If you have had a recent urinary catheter, prostate biopsy, or treatment for prostatitis, postpone the test by 4–6 weeks.
