About this test

Most of the testosterone circulating in your blood is locked to carrier proteins — chiefly sex hormone-binding globulin (SHBG) and albumin — and is not available to your tissues. Free testosterone measures only the small unbound fraction that is biologically active and can actually enter cells and do its work, which is why it sometimes tells a clearer story than total testosterone alone. In men, a doctor orders this when there are signs of low androgens: reduced libido, erectile difficulty, fatigue, low mood, loss of muscle bulk, or unexplained infertility, and especially when the total testosterone sits in a borderline range or when SHBG is likely to be altered by obesity, diabetes, thyroid disease, or liver problems. In women, it is requested for symptoms of androgen excess — irregular or absent periods, acne, hirsutism (unwanted facial or body hair), or scalp hair thinning — where polycystic ovary syndrome (PCOS) is the commonest cause and is frequently seen in young women across Kathmandu. The free fraction is rarely interpreted in isolation; your clinician reads it alongside total testosterone, SHBG, LH, FSH, prolactin, and often a morning fasting glucose or thyroid panel to locate the problem in the testes or ovaries, the pituitary, or the binding proteins themselves. Because levels swing with the time of day, recent illness, and sleep, a single low or high result is usually confirmed with a repeat morning sample before any treatment is started.

Preparation

A morning sample is strongly preferred — please come between 7 and 10 am, as testosterone is highest early in the day and falls as the day goes on. No fasting required. Avoid this test for a few days after an acute illness or strenuous exercise, and tell us if you take any hormone, steroid, or thyroid medication.
