About this test

Total Testosterone measures the overall amount of the principal male sex hormone circulating in your blood, counting both the small fraction that floats freely and the much larger portion bound to carrier proteins such as sex hormone-binding globulin (SHBG) and albumin. Although it is the defining hormone of the male body — driving muscle mass, bone strength, body hair, the deepening of the voice at puberty, sperm production, and libido — it matters in women too, where smaller amounts are made by the ovaries and adrenal glands. In men, doctors order this test when there are signs of deficiency: persistent tiredness, low sex drive, difficulty getting or keeping an erection, loss of morning erections, shrinking muscle, low mood, or unexplained infertility, and it is central to investigating delayed puberty in boys. In women it is requested when there is excess hair growth on the face or body, acne, irregular or absent periods, scalp hair thinning, or trouble conceiving — most often to look for polycystic ovary syndrome, a common cause of these symptoms among young women in Kathmandu. Because levels swing through the day and dip with acute illness, stress, and poor sleep, a single low morning result is usually confirmed on a repeat sample before any diagnosis is made. The number is rarely read alone; your doctor will interpret it alongside LH, FSH, SHBG, prolactin, and sometimes calculated free testosterone to work out whether the problem lies in the testes or ovaries themselves or in the pituitary gland that signals them.
