About this test

Thyroglobulin is a large protein made only by the cells of the thyroid gland, where it serves as the raw material from which thyroid hormones are built. Because healthy thyroid tissue and most thyroid cancers both release it into the blood, measuring it is rarely about diagnosing an overactive or underactive gland — for that, your doctor relies on TSH, T4 and T3. Its real value comes after treatment for differentiated thyroid cancer (papillary or follicular type). Once the gland has been removed by surgery and any remnant destroyed with radioactive iodine, there should be almost no source of thyroglobulin left, so the level ought to fall close to zero. A persistently detectable or rising figure on follow-up is the most sensitive early warning that some thyroid cancer cells remain or have returned, often long before they show up on a scan. This is why oncologists and endocrinologists order it at regular intervals for years after such treatment. The test is always interpreted together with an anti-thyroglobulin antibody measurement from the same sample, because these antibodies — common in autoimmune thyroid disease, which is frequent in Nepali women — can falsely lower the reading and make a true result look reassuringly clean when it is not. Levels are also compared against your own previous values rather than a single fixed cut-off, since the trend matters far more than one number. Occasionally it is checked to investigate the cause of an overactive thyroid or unusual neck swelling, but its chief role remains long-term cancer surveillance, where a stable low result offers genuine reassurance.

Preparation

No fasting required. If you take biotin (vitamin B7) or hair-and-nail supplements, please stop them 2–3 days before the test, as biotin can distort the result. Tell us if you have had thyroid surgery or radioactive iodine.
