About this test
The Free Thyroid Function Test measures the three core thyroid hormones — Free T3, Free T4, and TSH — in a single blood sample. Unlike total T3/T4, the "free" versions reflect only the biologically active fraction that the body actually uses, which makes them more reliable in pregnancy, in patients on oestrogen therapy, and in anyone with conditions that change protein levels. The combined reading lets the doctor confirm both the type of thyroid disorder (hypo- vs hyperthyroidism) and where the problem sits (thyroid gland itself or the pituitary). It is the standard follow-up test when TSH alone is abnormal, the recommended panel for patients already being treated for thyroid disease, and a useful test during the workup of unexplained weight change, infertility, irregular periods, palpitations, or persistent fatigue.
