About this test
This test looks for antibodies that the immune system mistakenly produces against the TSH receptor โ the docking site on thyroid cells that normally responds to the pituitary hormone TSH. When these antibodies are of the stimulating type, they switch the thyroid on continuously, driving the overactive gland of Graves' disease; less commonly, blocking antibodies switch it off and cause an underactive gland. Because of this, TRAB is the single most useful blood test for confirming that a patient's hyperthyroidism is specifically due to Graves' disease rather than a toxic nodule, thyroiditis, or other cause. Doctors order it when someone has a fast heartbeat, weight loss despite a good appetite, hand tremor, heat intolerance, anxiety, or the bulging, irritated eyes (thyroid eye disease) that are almost unique to Graves'. It is also requested to predict whether antithyroid medication can safely be stopped, since persistently high levels warn of relapse, and to assess the risk to the baby in a pregnant woman with current or past Graves' disease, as these antibodies cross the placenta and can affect the foetal thyroid. The result is always read together with the patient's free T4, free T3, and TSH, and often alongside an ultrasound or anti-TPO antibodies. A clearly raised value strongly supports Graves' disease, while a normal value in someone who is hyperthyroid points the doctor toward investigating other causes. The test does not require fasting and gives a clear, actionable answer that shapes both treatment and monitoring.
