About this test
TSH is the front-line screening test for thyroid disorders — by itself it tells the doctor whether your thyroid is overworking, underworking, or normal. The pituitary gland releases TSH to nudge the thyroid; if the thyroid is sluggish (hypothyroidism), the pituitary works harder and TSH rises, and if the thyroid is overactive (hyperthyroidism), the pituitary backs off and TSH falls. Symptoms that justify ordering this test include persistent fatigue, weight gain or loss without trying, hair thinning, irregular periods, mood changes, palpitations, or feeling cold all the time. We recommend it as part of every routine annual check-up for women over 30 — undiagnosed hypothyroidism is very common in Nepal. If TSH is abnormal, your doctor will often add Free T3 and Free T4 to clarify which type of disorder you have.
