About this test

The Absolute Lymphocyte Count tells your doctor exactly how many lymphocytes — the white cells that run your immune defence — are circulating in each microlitre of blood. Unlike the percentage figure on a routine report, the absolute number multiplies that percentage by your total white-cell count, giving a far more reliable picture; a normal adult usually sits between roughly 1,000 and 4,000 cells per microlitre. Lymphocytes include the B cells that make antibodies, the T cells that coordinate and kill infected cells, and natural-killer cells, so their number rises and falls with the state of your immune system. A high count (lymphocytosis) commonly accompanies viral infections such as influenza, glandular fever, hepatitis and COVID-19, and in older adults a persistently raised count can be an early clue to chronic lymphocytic leukaemia, prompting a peripheral smear and flow cytometry. A low count (lymphopenia) is what concerns clinicians most: it is seen with corticosteroid use, malnutrition, advanced tuberculosis — still widespread across Nepal — and notably with HIV, where this number helps gauge immune suppression before CD4 testing. Doctors often order it when someone has recurrent or unusually severe infections, unexplained weight loss, night sweats, or enlarged lymph nodes, and it is tracked closely in patients on chemotherapy or immunosuppressants to judge infection risk. The result is never read alone — it is interpreted alongside the rest of the complete blood count, the white-cell differential, and the clinical history, and a single abnormal value is usually repeated before any firm conclusion is drawn.
