About this test
The Activated Partial Thromboplastin Time measures how long your blood takes to clot through what is called the intrinsic and common coagulation pathway, and it is reported in seconds against a laboratory control. It is sensitive to clotting factors VIII, IX, XI and XII, as well as the shared factors X, V, II (prothrombin) and fibrinogen, so a prolonged result points your doctor towards a problem somewhere along this chain. Doctors most commonly order it before surgery or an invasive procedure, when investigating a patient who bruises easily, bleeds for an unusually long time after a cut or dental extraction, has heavy menstrual periods, or has a family history of a bleeding disorder such as haemophilia A or B or von Willebrand disease. It is also the standard test used to monitor patients on an unfractionated heparin drip in hospital, where the dose is adjusted to keep the APTT within a target range. A prolonged APTT does not by itself name the cause; it is interpreted alongside the Prothrombin Time (PT/INR), platelet count and, where needed, specific factor assays and mixing studies that distinguish a true factor deficiency from an inhibitor such as the lupus anticoagulant. For accurate results the citrate tube must be filled to the correct mark, since under-filling falsely prolongs the time. A normal APTT is reassuring before most operations, while an unexpectedly long value in an otherwise well person often simply reflects a harmless factor XII change rather than any bleeding tendency, which is why repeat testing and clinical correlation matter.

Preparation
No fasting required. If you take a blood-thinning medicine such as heparin or warfarin, tell us the name and your last dose, as it directly affects the result. Bring details of any unfractionated heparin infusion you are receiving.
