About this test
The Anti-Streptolysin O test puts a number on the antibodies your immune system makes against streptolysin O, a toxin released by group A Streptococcus — the bacterium behind strep throat and certain skin infections. Because these antibodies rise a week or two after the infection and linger for months, a quantitative ASO titre acts as a backwards-looking footprint, confirming a recent streptococcal exposure even after the throat has fully settled. Doctors order it mainly when they suspect a complication of untreated strep rather than the sore throat itself: acute rheumatic fever (joint pains, carditis, an unexplained heart murmur in a child), post-streptococcal glomerulonephritis (puffy face, dark or reduced urine, raised blood pressure after a recent throat or skin infection), Sydenham's chorea, or recurrent tonsillitis. In Nepal, where rheumatic heart disease remains a real cause of acquired heart disease in children and young adults, the ASO titre carries genuine weight in tracking down the source of migratory joint pains or a new murmur. A single raised value confirms past infection but does not date it precisely, so doctors often pair it with anti-DNase B, throat culture, inflammatory markers such as ESR and CRP, urine examination, and an echocardiogram when rheumatic fever is on the cards. A rising titre across two samples taken a fortnight apart is more telling than one isolated number, and a normal result helps reassure when streptococcal disease is unlikely.
