About this test

Direct bilirubin measures the conjugated fraction of bilirubin — the form your liver has already processed by attaching it to a sugar molecule so it can be excreted into bile. When the liver is healthy this water-soluble fraction stays low; a rise points specifically towards trouble downstream of the liver cell, rather than simply too much bilirubin being produced. Because of this, a raised direct bilirubin is one of the most useful clues a doctor has when sorting out the cause of jaundice (yellowing of the eyes and skin). It is the marker that distinguishes an obstructed bile duct, gallstones, or liver-cell damage from the harmless unconjugated rise seen in conditions such as Gilbert's syndrome or in the breakdown of red cells. The test is almost never read on its own: it is interpreted alongside total bilirubin (the indirect fraction is calculated by subtraction), the liver enzymes ALT, AST, ALP and GGT, and often an ultrasound of the abdomen. Doctors order it when a patient notices yellow eyes, dark tea-coloured urine, pale stools, itching, abdominal pain or unexplained fatigue, and to follow known liver disease. In Kathmandu it is requested frequently during episodes of viral hepatitis A and E, which spread through contaminated food and water and remain common here, as well as in alcohol-related liver disease and in newborns being assessed for prolonged jaundice. A persistently raised conjugated bilirubin always warrants prompt review by your doctor.
