About this test

Bilirubin is the yellow-orange pigment released when old red blood cells are broken down, and this test measures the total amount circulating in your blood — both the indirect (unconjugated) fraction that travels to the liver and the direct (conjugated) fraction that the liver has processed for disposal in bile. When the total climbs, the skin and the whites of the eyes take on a yellow tinge, the sign we call jaundice. A doctor orders it whenever that yellowing appears, or alongside dark urine, pale stools, itching, right-upper-abdominal pain, fatigue, or unexplained nausea. The pattern matters as much as the number: a raised indirect fraction points towards faster red-cell destruction (haemolysis), the harmless inherited Gilbert's syndrome, or the common newborn jaundice that paediatricians watch closely in the first week of life. A raised direct fraction instead suggests the liver itself is inflamed or that bile flow is obstructed — by gallstones, hepatitis, or a blockage of the bile ducts. In Nepal viral hepatitis A and E are frequent causes of acute jaundice, often spreading during the monsoon when water supplies are contaminated, so this test is a mainstay of evaluating any febrile patient who turns yellow. It is rarely read alone; your physician will weigh it against liver enzymes (SGPT, SGOT, alkaline phosphatase), the direct-bilirubin split, and a complete blood count to decide whether the problem lies in the blood, the liver cells, or the drainage system, and how urgently it needs attention.

Preparation

No fasting required for the total value alone, though doctors often order it within a liver panel that may ask for 8–10 hours of fasting. Drink water normally. Avoid strenuous exercise and alcohol the night before. If the sample looks unusual we may protect it from light, as bilirubin breaks down on exposure.
