About this test

Blood Urea Nitrogen measures the nitrogen carried in urea, the waste compound your liver produces when it breaks down dietary and body protein. Healthy kidneys filter urea out of the blood into the urine, so the level in your blood reflects how well your kidneys are clearing waste and, just as importantly, how well-hydrated you are. A doctor commonly orders BUN as part of a routine kidney check, before starting or while monitoring medicines that pass through the kidneys, during admission for almost any acute illness, and to follow patients with diabetes or high blood pressure — the two leading causes of chronic kidney disease in Nepal. It is rarely read on its own; it is almost always interpreted alongside serum creatinine, and the BUN-to-creatinine ratio helps your doctor tell whether a raised value points to dehydration, gastrointestinal bleeding or a high-protein intake rather than true kidney damage. Symptoms that may prompt the test include reduced or frothy urine, swelling of the ankles or face, persistent tiredness, nausea, loss of appetite or confusion. A raised BUN can follow dehydration from diarrhoeal illness — still common during the warmer months here — heart failure, or obstruction to urine flow, while a low value is usually harmless and may simply reflect a low-protein diet, pregnancy or liver disease. Because results shift quickly with fluid status, your doctor will read this number in the context of your symptoms, hydration and other kidney markers rather than in isolation.

No special fasting is needed for BUN alone, but if it is part of a renal panel or metabolic profile your doctor may ask for 8 hours of fasting. Drink water as usual unless told otherwise.
