About this test

Calcium does much more than build bone — it is essential for normal nerve transmission, muscle contraction, blood clotting, and heart-rhythm regulation. Serum calcium measures the level of calcium circulating in the blood and is one of the first tests ordered when investigating bone aches, recurrent fractures, kidney stones, muscle cramps, tingling around the mouth, and unexplained fatigue or low mood. It is also part of the standard workup for parathyroid disorders, vitamin D deficiency, suspected hyperthyroidism, and certain cancers that affect bone. Total serum calcium is often interpreted alongside albumin (because calcium binds to albumin) and ionised calcium for accuracy. Mildly abnormal readings are usually repeated and combined with phosphorus, vitamin D, PTH, and a 24-hour urine calcium for a complete picture before treatment is started.

Preparation

Fasting (8–12 hours) preferred. Avoid calcium or vitamin D supplements for 24 hours before the test as they can transiently affect the result.
