About this test

CK-MB measures the heart-specific fraction of creatine kinase, an enzyme released into the blood when muscle is damaged. While total creatine kinase comes mostly from skeletal muscle, the MB isoenzyme is concentrated in heart muscle, so a rise points specifically towards injury to the heart. Doctors order it when someone arrives with chest pain, breathlessness, sweating, or pain spreading to the arm or jaw, to help confirm or rule out a heart attack (myocardial infarction). After cardiac injury, CK-MB typically begins to rise within three to six hours, peaks around twenty-four hours, and returns to normal within two to three days — this last feature makes it especially useful for detecting a second, fresh heart attack soon after a first one, when troponin may still be elevated from the earlier event. It is almost always interpreted alongside troponin (the more sensitive marker), the ECG, and the clinical picture rather than on its own, and serial samples taken a few hours apart are far more informative than a single reading. A raised value is not always cardiac: vigorous exercise, intramuscular injections, muscle trauma, and certain muscle diseases can also lift it, which is why the result is often expressed as a ratio against total CK.
