About this test

Your two kidneys are filled with roughly a million tiny filters called glomeruli, and the Glomerular Filtration Rate is the single best number we have for how well that filtering machinery is working — essentially, how many millilitres of blood your kidneys clean every minute. In everyday practice it is not measured directly but estimated (eGFR) from your blood creatinine level combined with your age and sex using a validated formula such as CKD-EPI, so the result you receive is a calculated value reported in mL/min/1.73m². A figure of 90 or above is generally normal, while persistently lower numbers point towards chronic kidney disease, which is staged from 1 to 5 according to how far the GFR has fallen. Doctors order this test when screening anyone with diabetes or high blood pressure — the two leading causes of kidney failure in Nepal — and before prescribing or dosing medicines that the kidneys clear, including many antibiotics and contrast dye used in CT scans. It is also requested when symptoms such as ankle and facial oedema, foaming urine, persistent fatigue, poor appetite, or reduced urine output appear. Because creatinine depends on muscle mass, the eGFR can read falsely high in very thin or elderly people and is less reliable in pregnancy, so we interpret it alongside urine protein, blood urea, and electrolytes rather than in isolation. A single low reading is not a diagnosis; kidney disease is confirmed only when the reduction persists for at least three months, which is why your doctor will often repeat the test.
