About this test

Gamma-glutamyl transferase is an enzyme found in high concentrations in the cells lining the bile ducts of the liver, with smaller amounts in the kidneys, pancreas and gut. When these cells are irritated, inflamed or obstructed, GGT leaks into the bloodstream, which is why a raised level is one of the most sensitive early signals of liver and biliary-tract trouble. Doctors most often order it to investigate an elevated ALP (alkaline phosphatase) result: if both are high together, the problem is almost certainly arising from the liver or bile ducts rather than from bone, whereas a normal GGT alongside a high ALP points away from the liver. GGT is also the single most reliable laboratory marker of alcohol-related liver injury, rising steadily with regular heavy drinking and falling again over a few weeks of abstinence, so it is frequently used to monitor recovery. It commonly accompanies investigation of obstructive jaundice, gallstones, fatty liver, certain medication effects, and unexplained tiredness, nausea, abdominal discomfort or itching. Because it is so easily nudged upward by alcohol, some drugs and non-alcoholic fatty liver disease, GGT is rarely interpreted alone; it is read alongside ALT, AST, bilirubin and ALP to build a complete picture of liver function. In Nepal, where alcohol-related and fatty liver disease are increasingly common and where some traditional remedies and over-the-counter medicines can stress the liver, GGT is a valuable, inexpensive test for clarifying borderline liver results and tracking how the liver responds once the underlying cause is addressed.

Preparation

No strict fasting needed for the test itself, but please avoid alcohol for at least 24 hours beforehand, as even a single drink can push the result up. If a full liver panel is being done together, follow the fasting advice given for that.
