About this test
Globulin is not a single substance but a broad family of blood proteins โ including the alpha, beta and gamma fractions, the last of which contains your antibodies (immunoglobulins). Rather than being measured directly, total globulin is usually calculated by subtracting albumin from total protein on a liver or total-protein panel, and it is most informative when read alongside albumin as the albumin-to-globulin (A/G) ratio. Doctors look at globulin to understand two broad questions: is the liver making enough protein, and is the immune system behaving normally. A raised globulin level points towards chronic infection, chronic inflammation, autoimmune disease such as lupus or rheumatoid arthritis, or in older patients abnormal antibody production from conditions like multiple myeloma โ when myeloma is suspected, protein electrophoresis is the natural next step. A low globulin can reflect poor antibody production, certain kidney diseases where protein is lost in the urine, or impaired liver synthesis. In Nepal, where chronic infections such as tuberculosis, kala-azar and long-standing hepatitis B and C remain common, a persistently high globulin with a reversed A/G ratio is a frequent and clinically useful clue. The test is commonly ordered when someone has unexplained weight loss, recurrent or lingering infections, swelling, fatigue, or an abnormal total-protein result found on a routine check. On its own a globulin value rarely gives a diagnosis; its real value is in steering your doctor towards the right group of follow-up tests.
