About this test
When the heart muscle is stretched by extra pressure or volume, it releases a hormone called BNP, and NT-pro BNP is the stable fragment left behind when that hormone is made — making it one of the most useful blood markers we have for detecting strain on the heart. Doctors order it chiefly to confirm or rule out heart failure in someone who arrives breathless, with swollen ankles, unexplained fatigue, or difficulty lying flat at night. Its real strength lies in a reassuringly low result: a normal value makes heart failure very unlikely, which helps us avoid unnecessary scans, while a clearly raised level points us towards an echocardiogram and prompt cardiology review. Beyond diagnosis, the test is valuable for tracking treatment — falling numbers usually mean the heart is responding to medication, and rising numbers may signal worsening or fluid overload. Interpretation is never done on the number alone; we read it alongside the patient's symptoms, an ECG, chest X-ray, kidney function and echocardiogram, because levels naturally climb with age and in kidney impairment, and may be lower in people who carry excess weight. In Kathmandu we see this picture often in older patients with long-standing high blood pressure or rheumatic heart disease, where breathlessness can easily be mistaken for a chest infection at altitude. The test requires no fasting and uses a single tube of blood, giving your doctor an objective, same-day measure of how hard your heart is working and whether the breathlessness deserves a closer cardiac look.
