About this test

Serum phosphorus measures the level of inorganic phosphate in the blood. Phosphorus works closely with calcium to build bone and is essential for energy metabolism (it's the P in ATP), so abnormal levels usually point to problems in the kidney, the parathyroid glands, or the gut. Doctors order this test when bone or muscle complaints have already pushed them to check calcium, when investigating recurrent kidney stones, in known chronic kidney disease (where phosphorus retention is one of the biggest day-to-day management problems), in alcoholism and malnutrition, and after refeeding seriously ill patients (low phosphorus can be dangerous). Children with rickets or growth failure also need a phosphorus level alongside calcium, alkaline phosphatase, and vitamin D. It is rarely ordered alone — interpretation is much easier with calcium, magnesium, and renal function alongside it.

Preparation

Overnight fasting (8–12 hours) is preferred — phosphorus rises after meals, especially carbohydrate-heavy ones, which can give a falsely high reading.
