About this test

Serum protein electrophoresis separates the proteins circulating in your blood into bands according to how they move through a gel under an electric current, producing a tracing with five main zones: albumin and the alpha-1, alpha-2, beta, and gamma globulin fractions. Rather than giving a single number, it shows the relative balance between these groups, and the pattern itself often points to a diagnosis. A doctor typically orders this test when there is unexplained anaemia, persistent bone pain, kidney impairment, a very high ESR, frequent infections, or abnormal total protein and albumin-globulin ratio on routine biochemistry. Its most important role is detecting a monoclonal band (M-spike) in the gamma region, which raises suspicion of multiple myeloma or a related plasma-cell disorder, usually followed by immunofixation and free light-chain studies to confirm the abnormal antibody. The tracing also reveals broad polyclonal increases in gamma globulins seen in chronic infections, autoimmune disease, and liver cirrhosis; a low albumin with raised alpha-2 globulin suggesting nephrotic syndrome where protein is lost in urine; and reduced gamma globulins indicating an immune deficiency. In Nepal, where chronic infections, tuberculosis, and untreated liver disease are common, a reactive polyclonal pattern is frequently seen and must be carefully distinguished from the sharp, sinister monoclonal spike. Results are always read alongside your symptoms, full blood count, renal and liver function, calcium, and, when needed, specialised follow-up testing rather than interpreted in isolation.
