About this test

The qualitative RA Factor test screens your blood for rheumatoid factor, an autoantibody that the immune system mistakenly produces against its own immunoglobulin (IgG). The test is done by a rapid latex agglutination method that reports a simple positive or negative result, usually within the same day. Doctors most commonly order it when a patient complains of joint pain that has lasted several weeks — particularly symmetrical swelling, stiffness, and warmth in the small joints of the hands, wrists, or feet, and morning stiffness that takes more than half an hour to ease. A positive result lends support to a diagnosis of rheumatoid arthritis, the most frequent reason this test is requested, but it is never read in isolation. Rheumatoid factor can also turn positive in other autoimmune conditions such as Sjogren syndrome and lupus, in chronic infections, in long-standing tuberculosis (still common in Nepal), in chronic liver disease, and even in a small proportion of perfectly healthy older adults. Equally, some people with genuine early rheumatoid arthritis test negative, so a negative result does not rule the disease out. Because of this, your physician will weigh the result against your symptoms, joint examination, and other tests such as anti-CCP antibodies, ESR or CRP for inflammation, and sometimes joint X-rays. If the qualitative screen is positive, a quantitative (titre) RA Factor is often advised next to measure how high the level is. Think of this test as a useful first signpost on the path to diagnosis rather than a final verdict on its own.
