About this test

Rheumatoid factor is an antibody that the immune system mistakenly directs against the body's own immunoglobulins, and this quantitative version reports the exact concentration in international units per millilitre (IU/mL) rather than a simple positive-or-negative answer. The number matters because higher titres tend to track with more aggressive joint disease, so your doctor can use it both to support a diagnosis and to gauge severity. The commonest reason for ordering it is suspected rheumatoid arthritis, particularly when someone has symmetrical pain, swelling and morning stiffness lasting over an hour in the small joints of the hands, wrists or feet. It is important to understand that a raised RA factor is not proof of rheumatoid arthritis on its own, and a normal result does not rule it out, because roughly one in five patients are seronegative. For that reason it is almost always interpreted alongside anti-CCP antibodies, ESR, CRP, a complete blood count and the clinical picture. A modestly elevated level can also appear in Sjogren syndrome, lupus, chronic liver disease, and longstanding infections, and it rises naturally with age in some healthy people. In Nepal, where chronic infections such as tuberculosis and hepatitis are not uncommon, this overlap is worth bearing in mind before a result is taken as confirmation of arthritis. Early identification of rheumatoid arthritis genuinely changes outcomes, since prompt treatment protects joints from permanent damage, making this a useful first-line investigation for persistent, unexplained joint symptoms.
