About this test

SGPT, now more correctly called ALT (alanine aminotransferase), is an enzyme that lives mainly inside liver cells, where it helps the body process amino acids. When liver cells are inflamed or injured, this enzyme leaks into the bloodstream, so a raised ALT is one of the most sensitive and specific early signals that something is irritating your liver. Because ALT is far more concentrated in the liver than in any other organ, it is the single most reliable blood marker your doctor uses to ask the question: is the liver itself in trouble? It is requested when you have symptoms such as yellowing of the eyes or skin (jaundice), unexplained fatigue, nausea, dark urine, pale stools, or discomfort under the right ribs, and it is routinely checked before and during treatment with medicines that can stress the liver, including TB drugs, certain cholesterol tablets, and long-term painkillers. In Nepal, ALT is commonly ordered to investigate viral hepatitis A and E (often picked up from contaminated water, especially during the monsoon), hepatitis B and C, alcohol-related liver disease, and the increasingly common fatty liver linked to diabetes and obesity. It is rarely read in isolation: doctors interpret ALT alongside SGOT/AST, bilirubin, alkaline phosphatase, GGT, and albumin, and the ratio between ALT and AST can hint at the underlying cause. A single mildly raised value is usually nothing to fear and is often rechecked after a few weeks to confirm a genuine, lasting trend.

Preparation

No fasting is strictly required, but your doctor may ask you to fast if SGPT is being measured as part of a liver panel or with a lipid profile. Avoid alcohol for 24 hours before the test, and mention any painkillers, antibiotics, herbal tonics, or paracetamol you have taken recently.
