About this test
Total Iron Binding Capacity is an indirect measure of transferrin, the protein that ferries iron through the bloodstream, and it tells your doctor how much spare capacity your blood has to bind and transport more iron. The body produces extra transferrin when iron stores run low, so TIBC characteristically rises in iron-deficiency states even before haemoglobin falls โ which is precisely why it is so useful. It is rarely interpreted on its own; instead it is read alongside serum iron and ferritin, and together these allow calculation of transferrin saturation, the percentage of binding sites actually carrying iron. A high TIBC with low serum iron and low saturation points firmly to iron deficiency, while a low TIBC may reflect chronic illness, infection, inflammation, liver disease, malnutrition or, less commonly, iron overload. Your doctor may order it when a complete blood count shows small, pale red cells suggesting iron-deficiency anaemia, or when you report persistent tiredness, breathlessness on exertion, pallor, brittle nails or unusual cravings such as eating ice or soil. In Kathmandu this picture is common: iron-deficiency anaemia affects a large share of menstruating women, pregnant mothers and people following predominantly vegetarian diets where dietary iron is less readily absorbed, and intestinal parasites can quietly worsen iron loss. The test is also used to monitor response once iron treatment begins. Because results are sensitive to recent supplements and the time of day, following the simple fasting and timing instructions helps ensure your numbers are genuinely meaningful.
