About this test

Total protein measures the combined amount of all the proteins circulating in your blood, made up chiefly of two groups โ€” albumin and the globulins. Albumin is manufactured by the liver and keeps fluid inside your blood vessels, while the globulins include antibodies and many transport and clotting proteins. Because these two move in different directions in different illnesses, the laboratory usually reports the total figure alongside albumin and a calculated albumin-to-globulin (A/G) ratio, which often tells the doctor more than the total alone. The test is ordered as part of a routine liver function panel, during the assessment of unexplained weight loss, swelling of the legs or abdomen (oedema and ascites), persistent fatigue, or recurrent infections, and to help work up suspected kidney disease where protein is lost in the urine. A low total protein may point to poor nutrition, liver disease, or protein-losing conditions of the gut or kidney; in Nepal this is worth noting in strictly vegetarian diets, chronic diarrhoeal illness, and in people with longstanding alcohol-related or hepatitis-B liver disease. A high total protein can reflect dehydration or, more importantly, an excess of globulins seen in chronic infection, certain inflammatory disorders, and bone-marrow conditions such as multiple myeloma. On its own the number is rarely diagnostic, so an abnormal result usually leads on to further testing โ€” serum albumin, liver enzymes, kidney function, or protein electrophoresis โ€” to find the cause. It is a simple, inexpensive, and useful first step in a broad range of investigations.
