About this test
TPHA detects antibodies that the body makes specifically against Treponema pallidum, the spiral-shaped bacterium that causes syphilis. Unlike the older VDRL and RPR tests, which measure non-specific reagin antibodies and can occasionally turn positive in pregnancy, certain infections, or autoimmune conditions, TPHA targets the organism itself, so it is used as the confirmatory step after a reactive screening result. In the qualitative form we are simply answering one question: are treponemal antibodies present or absent. A reactive result strongly indicates either a current or a past syphilis infection, because treponemal antibodies usually remain detectable for life even after successful treatment; this is why TPHA is excellent for confirming infection but cannot, on its own, tell you whether the disease is active or already cured. For that reason your doctor will read it alongside a VDRL or RPR titre and your clinical history. The test is commonly requested when a screening test is positive, when there are genital ulcers, an unexplained body rash involving the palms and soles, during the routine antenatal panel to protect the baby from congenital syphilis, before blood donation, and as part of a sexual-health check-up that may include HIV and hepatitis B and C screening. In Nepal, antenatal syphilis screening is part of standard pregnancy care, and TPHA is the dependable confirmatory tool when an initial test flags a concern. A positive result is treatable; syphilis responds very well to penicillin, so confirmation simply opens the door to straightforward treatment and follow-up.
