About this test

Triglycerides are the main form in which the body stores and transports fat, and this test measures how much of them is circulating in your blood after an overnight fast. Whenever you eat more calories than you burn — particularly from sugar, refined carbohydrates such as rice, alcohol, and fried food — the excess is converted into triglycerides and carried in lipoprotein particles. A doctor usually orders this test as part of a lipid screen to gauge cardiovascular risk, but it carries information the cholesterol numbers do not: a high result points towards insulin resistance, the metabolic syndrome, type 2 diabetes, an underactive thyroid, kidney disease, or simply heavy alcohol intake. It is checked routinely in anyone overweight, with a strong family history of diabetes or heart disease, or already on treatment for high lipids, and it is rechecked to see whether diet, exercise, or medication is working. Most people have no symptoms, which is why testing matters; very high levels (typically above 500 mg/dL) are a separate concern because they can trigger acute pancreatitis, which presents with severe upper-abdominal pain and vomiting. Borderline elevations are extremely common in Kathmandu, where rice-heavy diets, sedentary office work, and rising rates of diabetes combine to push levels up quietly over years. Results are always interpreted alongside HDL, LDL, fasting glucose, and your overall risk picture rather than in isolation. Because levels swing widely with the last meal, the fasting instruction is genuinely important — a non-fasting sample can give a misleadingly high reading and lead to unnecessary worry.

Preparation

9–12 hours of overnight fasting is required, as recent meals can raise the result several-fold. Plain water is allowed. Avoid alcohol for at least 24 hours and skip any unusually fatty meal the night before.
