About this test
Troponin I is a protein found almost exclusively inside heart-muscle cells, and it leaks into the bloodstream when those cells are injured โ most importantly during a heart attack. This rapid immunochromatographic (ICT) version gives a quick qualitative yes-or-no answer at the bedside or in the emergency room, which is invaluable when a patient arrives with chest tightness, breathlessness, sweating, or pain radiating to the arm, jaw, or back and a decision must be made fast. A doctor orders it whenever a heart attack (myocardial infarction) is suspected, and it is the single most specific blood marker we have for cardiac muscle damage, far more reliable than older markers such as CK-MB. Troponin begins to rise roughly three to six hours after the heart is injured, which is why a single early negative result does not fully rule out a problem โ your doctor will usually repeat the test after a few hours and read it together with your ECG, symptoms, and risk factors. Raised troponin can also appear in heart failure, myocarditis, severe infection, kidney disease, and pulmonary embolism, so the result is always interpreted in context rather than in isolation. In Kathmandu, where cardiovascular disease is rising sharply and patients often reach hospital hours after symptoms start, a same-day troponin alongside an ECG is a frontline tool for separating a true cardiac emergency from less serious causes of chest pain. A positive ICT result should always prompt urgent cardiology review and, where available, a confirmatory quantitative high-sensitivity troponin assay.
