About this test

This rapid antibody test looks for two classes of antibody that your immune system makes against Salmonella Typhi, the bacterium that causes typhoid (enteric) fever. IgM antibodies appear first, usually from around the end of the first week of illness, and signal a current or very recent infection; IgG antibodies rise later and tend to persist for months, so on their own they may simply reflect a past infection or earlier vaccination. A doctor typically orders this test when a patient has prolonged fever lasting several days, headache, abdominal discomfort, poor appetite, constipation or loose motions, and general weakness — a picture that is extremely common in Kathmandu, where typhoid remains endemic and surges during and after the monsoon when water and food contamination peaks. Because it gives a same-day answer from a small blood sample, it is a practical bedside aid, but it is not perfect: antibodies take time to develop, so a test done in the first two or three days of fever can be falsely negative, while past exposure can produce a misleadingly positive IgG in our high-prevalence setting. For these reasons your clinician will read the result alongside your symptoms, the day of illness, a complete blood count, and, where possible, a blood culture, which is still the most definitive way to confirm typhoid and to guide antibiotic choice given rising drug resistance. If the test is negative but fever continues, repeat testing or further investigation for dengue, scrub typhus or other causes is often advised.
