About this test

The VDRL titre is a quantitative blood test for syphilis, the infection caused by the bacterium Treponema pallidum. Rather than detecting the bacterium directly, it measures antibodies your body makes against cardiolipin, a substance released when cells are damaged by the infection — and it reports the result as a titre, the highest dilution (for example 1:2, 1:8, 1:32) at which the blood still reacts. That number matters: a high or rising titre points to active, untreated disease, while a falling titre after treatment is the clearest laboratory proof that the antibiotics are working. Doctors order it to screen during pregnancy (untreated maternal syphilis can cause stillbirth or congenital infection in the baby), before blood donation, in anyone with genital ulcers, an unexplained rash on the palms and soles, or a new sexually transmitted infection, and routinely alongside HIV testing since the two often travel together. Because it is a non-treponemal test, it can occasionally give a false-positive reaction in pregnancy, after some viral illnesses, in tuberculosis, or in autoimmune conditions such as lupus, so a reactive VDRL is always confirmed with a treponemal-specific test (TPHA or FTA-ABS) before a diagnosis is made. In Nepal it remains a standard part of antenatal and pre-marital screening, and is widely used to monitor patients through their course of penicillin therapy. A non-reactive result is reassuring, though very early infection may not yet show, in which case repeat testing after a few weeks is advised.
