About this test
The Widal test screens for antibodies against the Salmonella Typhi and Paratyphi bacteria, the cause of typhoid fever โ a very common diagnosis in the urban Kathmandu monsoon season. The test measures four titre values (TO, TH, AO, AH) which together help a doctor decide whether the antibody response is consistent with an active typhoid infection. Because antibodies take 7โ10 days to rise, a single Widal test in the first few days of fever can be falsely negative; doctors usually repeat it after 5โ7 days and look for a rising titre. Widal results are interpreted alongside the clinical picture โ high fever lasting more than 4โ5 days, abdominal pain, headache, and constipation are typical โ and ideally confirmed by a blood culture, which remains the gold standard for diagnosing typhoid.
